Exceptional Leave During Term Time Form
If there is a change to your child's medical information, please complete the below google form.
Should your child require medicine to be administered during the school day, please complete the below form. This must be handed to, or emailed to the school office. Please note, if we do not have parental permission, we will not be able to administer any medicines.
Parental Permission to Give Medicines
All Medicines must be handed into School Staff, they must not be kept by children. School staff will hand medication back to a parent or carer at the end of the school day.
